HOW TO APPLY FOR A MEDICAL CARD:

Call in to collect an application form from one of our three surgeries, or application forms can be downloaded through the HSE website.(For this option, download and complete the form, then bring to our surgery for signing) You can apply online or check the status of your medical card application by clicking the following link: http://www.medicalcard.ie/

Newborn Babies  In order for your newborn baby to be added to your medical card, you will need to apply for a medical card for them. We can help with this. For this we will need your babies date of birth and PPS number. If you require assistance , please contact Shiona our Practice Manager by phoning our surgery.

European Health Insurance Card The European Health Insurance Card or EHIC (fornerly the E111 form) allows the holder to access health care services when travelling to other EU or EEA countries. If you wish to apply for the European Health Insurance Card, this can be done through the HSE website or Alternatively by contacting the HSE local offices by phone on 028 40400, or by calling in to The administrative offices , in Coolnagarrane, Hospital Grounds, Skibbereen

    MEDICAL EMERGENCY: DIAL 999 if  you suspect a stroke or heart attack

    In the event of a  medical emergency, please inform our secretary of the patients name, location and your contact telephone number. You will be transferred to a doctor or we will return your call promptly

    Mizen Medical Practice operates a strict no interruption policy during standard surgery hours (except in the case of a medical emergency) to ensure that the patient receives an uninterrupted consultation with their GP or Nurse.
